L. J., a male, was admitted to the hospital at age 11 years with cough, fever, easy fatigability, and loss of greater than 5 lb of weight. Five months prior to this admission, he had a flu- like illness which was followed by pneumonia 1 week later. He was treated by his local physician with IM penicillin, followed by oral penicillin with apparent improvement; however, he continued to cough. The cough was nonproductive with no hemoptysis. Review of his past history was negative except for occasional mild upper respiratory infections.
